Primitive Reflexes: What They Are and Why Therapists Check Them
Every baby is born with a set of automatic movement patterns — primitive reflexes — that usually fade during the first year of life. When they don't, research links them to differences in attention, coordination, and learning. Here's the plain-English version.
What Are Primitive Reflexes?
Primitive reflexes are involuntary movements babies are born with — like the startle (Moro) reflex, the rooting reflex that helps with feeding, and the tonic neck reflexes that link head position to arm movement. They're run by the brainstem, and they exist to help newborns survive and develop before higher brain regions mature.
As a baby's brain develops, these automatic patterns are normally "integrated" — replaced by voluntary, controlled movement. Most fade within the first 6–12 months.
What Happens when Reflexes Are Retained?
Sometimes reflexes persist beyond infancy — what therapists call retained primitive reflexes. Research has repeatedly found associations between retained reflexes and developmental differences. Studies we summarize on our Research page report, for example, that retained tonic neck reflexes are consistently linked with ADHD symptoms, and that reflex patterns tracked with measurable differences in brain connectivity.
An important caution: an association is not the same as a cause. A retained reflex doesn't mean a child will have difficulties, and integrating one isn't a guaranteed fix. What retained reflexes offer clinicians is a window into how a child's nervous system organized its foundations.
Why Do Therapists Check for Them?
Because they're observable and workable. A therapist can assess reflexes with simple, non-invasive movement checks, and — where retained patterns show up — build exercises that give the nervous system the movement experiences those reflexes were waiting for. That's the idea behind reflex integration therapy.

What Might Parents Notice at Home?
Signs that sometimes prompt an evaluation include:
- A child who is bright but fatigues quickly when writing or reading
- Persistent clumsiness or difficulty with coordination milestones
- Strong startle responses or ongoing difficulty with self-regulation
- W-sitting, poor posture at a desk, or avoiding crossing the body's midline
None of these prove a retained reflex — they're simply reasons a whole-child evaluation can be informative.
